Access to Healthcare

The Health Collaborative is working to increase access to healthcare for individuals and families in our region without a primary care provider or medical home. One strategy to address these healthcare needs is the emerging Regional Engagement to Advance Community Health (REACH) Partnership of the Dan River Region - the next iteration of the Community Health Worker Project . The Collaborative is also exploring strategies to train and orient clinical practices to address healthy weight management, develop a system to provide 24-hour access to medical advice and create a taskforce of providers and healthcare leaders focused on population health and chronic disease prevention. These strategies will focus on access to healthcare in the communities of highest need, while identifying and teaching residents how to manage their own health issues.

Imagine a Dan River Region where:
  • Healthcare providers deliver consistent prevention recommendations and work together with regional partners to connect patients with community resources
  • All residents have a primary care provider to receive clinical preventative services, such as screenings and immunizations and have access to resources that support healthy decisions.
